A 56-year-old female with no known past medical history is brought to the ED (ER) with a sudden onset of severe chest pain. You cannot take further history of her present illness because she is barely able to talk due to severe pain. On physical examination she appears very anxious and her BP is 160/84 and pulse is 104. Her breathing is slightly exertional but the remainder of her physical exam was normal. A resident who evaluates the patient thinks she is having acute myocardial infarction and indeed her electrocardiogram reveals ST elevations on some leads. The resident orders CK with isoenzymes and troponin I in addition to chemo profiles and CBC. 5 minutes later, the resident notices that the patient is barely able to move her legs.
