A 65-year-old diabetic male has driven alone all the way from Texas to California, taking more than 12 hours and having only one bathroom rest. On arrival in California, he begins to have sharp chest pain and shortness of breath. Physical examination reveals a slightly anxious man with a respiratory rate of 20 per minuite and a swollen, erythematous and tender right calf.
What tests would you order to confirm your preliminary diagnosis? What treatments would you give for the most likely diagnosis?
Deep vein thrombosis in the right calfによるpulmonary embolismがmost likelyなので、まずはultrasoundでright calfにおける血栓の有無をチェックし、spiral CTでpulmonary embolismの有無を見ます。
From the clinical picture which satisfies one of the Virchow's triad, most likely diagnosis is deep vein thrombosis. For that, I agree with other people.
I order color doppler ultrasound of lower extremeties, which I think is enough to confirm DVT without any further tests on lungs.
Besides, it's always important to rule out MI, aortic dissection, PTX, and esophageal rupture, I order chest CT and EKG.
Treatmet will be IV heparin and later change it to po warfarin.
