Hi, my name is <<YOUR NAME>>. I am <<YOUR KID’S NAME>> mother. For returning children and parents, welcome back, and I hope you all had a great summer. For the new ones, welcome to the Classroom 1. You might be anxious for your child(ren), but this class is great and all the teachers are wonderful so you have absolutely nothing to worry about!
As the new school year begins, I want to let you know of my son’s food allergies:
<<YOUR KID’S NAME>> is severely allergic to peanuts, tree nuts, dairy, and eggs. He is now 4 years old and well aware of his food allergies. However, there are times that he does not know his friends just had PB&J sandwiches and if he has contact with them, he will likely have an anaphylactic reaction.
<<SCHOOL NAME>> is a peanut/tree nut free school and we are asked not to bring anything that contains peanuts and/or tree nuts. Please do remember this policy, especially when your child(ren) is Person of the Day and brings snacks/treats for the class. There is a snack list available that suggests good snacks and what should not be brought to school.

XXX allergic to Dairy Products (any types of cheese,
yogurts, chocolates ,etc.),
Wheat, Soy, Oats, Beans, Nuts, Eggs, Sesame ,and
Buckwheat.
XXX will bring her own snack every day.
Please let me know in advance if someone will have a
birthday parties or something like that you’ll be
giving other kids some special desserts.
I can bring XXX desserts on the designated dates.
XXX doesn’t understand English yet.
Allergic reaction that we have seen to the food listed
above:
-puffy cheeks and eyes
-swollen face
-extreme itchiness
-hives
-vomiting
-asthma attack
If XXX has severely hives or asthma attack, she have to go to the doctor immediately.
XXX has eczema from atopic dermatitis.
She always feels itchy .
If she can’t stop scratching her skin
-distract her(give toys, etc.)
-wash with cold water
-cool the itchy places by applying an ice pack
-apply Vaseline
We think she feels itchy after dirtying her skin (eating something or using Play-Doh or
playing sandbox etc.)
Would you mind washing her hands after these kids of
activities?
