We are writing in relation to your application for a working holiday visa, which was lodged on 24 December 2010.
We have received information that may be prejudicial to the approval of your application. This information is as follows:
Upon submitting your online application, you would have received an automated message requesting for a Temporary Entry Chest X-ray Certificate (NZIS Form 1096) to be completed by a listed panel doctor and provided within 30 days.
This is in accordance with Immigration Operational Manual policy reference A4.25.1, which states as follows - a. Applicants for temporary visas or permits who intend to be in New Zealand for more than six months, and not more than 12 months, and are considered to have risk factors for pulmonary tuberculosis must undergo a TB screening X-ray unless: i they are pregnant; or ii they are under 11 years of age.
A4.25.5 Definition of person with risk factors for pulmonary tuberculosis a. A person is considered to have risk factors for pulmonary tuberculosis if: i they hold a passport issued by a country not listed in Appendix 9; or ii in the five years prior to application they have spent a combined total of three or more months in any one or more countries not listed in Appendix 9.
Please provide a completed Temporary Entry X-ray Certificate (NZIS 1096) by 4 February 2011. Failure to respond to this opportunity within the specified time will result in a decision being made on the information currently held.
All supporting documents must be original or certified copies of originals and, if not in English, must be translated into English by a recognised and independent translation service.
What happens if you do not send any comments or additional information? If you do not send any comments or information by the date requested above we will make a decision on your application based on the information you have already given us. We are unlikely to approve your application based on this information.
False or misleading information Providing false or misleading information or withholding information may make you ineligible for a visa.
What happens if your circumstances change? You must tell us about any changes to your circumstances that may affect your application for a visa, including:
* changes to your address or contact details (including postal address, email address, and telephone number) * changes to your business or employment * changes to your course of study if you are applying for a student visa.
If you do not tell us about changes to your circumstances, we may decline to grant you a visa or you may become liable for deportation. While you are in New Zealand, you must make sure you hold a valid visa at all times.
*Immigration Act 2009 Immigration New Zealand (INZ) has changed the terminology it uses. From 29 November 2010, we will use the term visa to mean the authority to travel to New Zealand and stay in New Zealand. We have made this change because of the new Immigration Act 2009. If you applied for a permit before 29 November 2010, any correspondence sent by INZ will now refer to a visa. This change will not affect your immigration status.
